Primary Health
The aim of Primary Health is to support and promote the health and well-being of the community through health promotion, illness prevention and early intervention.
We can provide assessment, referral and information on a range of health and well being issues such as :
- Diabetes
- Heart Health
- Social Activities
- Healthy eating
- Physical Activity - such as Lifeball and Polewalking
- Asthma Awareness
- Mobility Problems
- Falls Prevention
- Foot care
- Mental health
- Adolescent Issues
- Women’s Health
- Men’s Health
- Management of chronic airways diseases

We also facilitate "Club Connection" for those adults who may need some addtional support to remain active and enjoy social activites away from their home.
We provide services to meet the needs of the communities of:
- Beechworth
- Silver Creek
- Stanley
- Wooragee
- Yackandandah (limited services provided in Yackandandah)
Who can access our services?
All members of the community can access the varied services available through the Primary Health Team. However eligability criteria applies to Club Connection. Please contact us if you have any queries and discover how we are able to assist you.
Where are we?
Allied Health and Community Health services are located in the building fronting the main road at 52 Sydney Road Beechworth . Some services also provide assessments at home (Occupational Therapy) or in community settings (Youth Counseling).
Beechworth Club Connection meets most Monday to Friday (9 days per fortnight) at 52 Sydney Road, Beechworth. Yackanadandah Club Connection meets on Monday’s and Thursday at the Senior Citizens Centre, Wellsford Street , Yackandandah and Wednesday at Yackatoon Village Hall, Butson St,Yackandandah.
No referral needed.
The Primary Health Team Services are all accessible through what is called a self referral. That means no GP referral is required. Please contact us if you have any queries and discover how we maybe able to assist you.
Administrative fees
A small administrative fee is changed for some services provided by the Primary Care Team, however service will not be refused due to an inability to pay this fee. Please contact us if you have any queries and we can assist you.
