Beechworth Health Service District Nursing Service (DNS)

This service is provided in the patients' own homes.

 

Services Provided:

  • Wound Management
  • Medication Management
  • Blood Pressure Monitoring
  • Management of Diabetes
  • Blood Sugar Monitoring
  • Post Operative Care
  • Palliative Care
  • Liaison with Doctor
  • Liaison with and referral to Allied Health Services
  • Assessment for access to Home Care and Meals-on-Wheels
  • Post Acute Care
  • Hospital in the Home
  • Continence Management
  • Assessments
 

Clients can access  our District Nursing Services via a number of means including:

  • Self referral
  • Referral by an outside provider including GPs, the District Nurse, Community Nurse, Shire or a friend.

On referral, a District Nurse will assess the prospective patient for eligibility for the program. Following consultation with the patient the nurse will determine which programs are most suitable. Service is initially offered on a trial basis.
